What the diaper output tracker checks

Enter your baby's age in months, how many wet diapers and stools they have in a day, and whether you have noticed any warning signs. The tracker gives a short hydration check and tells you when to contact your pediatrician.

It looks at the pattern in one day. A single odd day is usually not a concern, but a clear change deserves a call.

    How it works

The tracker applies these rules in order. Every rule that matches adds one line to your result.

    RuleMessage shown
Fewer than 4 wet diapers a dayLow wet diaper count may suggest hydration risk.
4 or more wet diapers a dayWet diaper count is usually within a reassuring range.
Under 6 months and no stoolsNo stool can be normal in some breastfed babies, but watch comfort and feeding.
6 months or older and no stoolsReview fiber/fluid pattern if stool frequency drops.
5 or more stools a dayFrequent stools may need hydration monitoring.
Warning signs tickedBecause warning signs were selected, contact pediatric care promptly.
Warning signs not tickedIf mood, feeding, or hydration changes abruptly, seek pediatric advice.

The cut-off of fewer than 4 wet diapers is a simple screening line used by this tool, not an official limit. The AAP lists fewer than six wet diapers a day in infants as a sign of dehydration, and expects 6 or more a day in breastfed babies from about 5 to 7 days old. If your count is low for your baby, do not wait for the tool to say so.

    Worked example

Baby aged 5 months with 6 wet diapers and 2 stools, no warning signs. The tool shows "Wet diaper count is usually within a reassuring range." and "If mood, feeding, or hydration changes abruptly, seek pediatric advice."

    Frequently asked questions

How many wet diapers a day is normal?

The AAP expects 6 or more wet diapers a day in breastfed babies by 5 to 7 days old. The tracker flags fewer than 4, but fewer than 6 is worth a call to your pediatrician if it is new for your baby.

Is it normal for a baby to skip a day of stools?

It can be, especially in some breastfed babies under 6 months, if they are feeding well and comfortable. Older babies who skip stools may need a look at fiber and fluids.

What counts as a warning sign?

Examples are very few wet diapers, a sunken soft spot, no tears when crying, a dry mouth, or being very sleepy or hard to wake. Tick the box if you see any of these.

    Can this tool diagnose dehydration?

No. It is an educational check. Only a health professional can assess hydration.

    Safety first

This tool is educational and does not diagnose anything. Call your pediatrician promptly, or seek urgent care, if your baby has very few wet diapers, a dry mouth, sunken eyes or soft spot, is unusually sleepy, or refuses feeds.
